What Is Perineal Massage?

Perineal massage is a manual technique applied to the perineum. It can be used during pregnancy and/or postpartum. For the purpose of this blog post we will focus on perineal massage during pregnancy.

 When to do it?

·      During pregnancy research has found perineal massage to be effective in reducing grade 3 and 4 perineal tears in first time moms.

·      It is generally recommended to begin perineal massage between 32 and 35 weeks and to perform it up to 3x a week until the baby arrives

·      Perineal massage can be done digitally or with a pelvic wand.

·      We always recommend using lubricant to reduce friction at the tissue.

Are you experiencing pain?

·      Perineal Massage should never be painful. If you are experiencing pain, we recommend seeing a pelvic PT to help guide your treatment

·      Perineal massage is a general technique only addressing the outermost layer of the pelvic floor muscles (the skin between the anus and vaginal opening). Thus it should not be used to treat specific pain or underlying pelvic floor dysfunction. For specific treatment for pain please see a Pelvic PT

·      If perineal massage is triggering in any way please discontinue use, there are many other ways to help address pelvic floor pain, dysfunction and to help support your body during labor and delivery. Check out our pregnancy prep class for more details.

 Why does it matter if I tear?

·      Tearing to the perineum (grades 2-4) include the outermost layer of the pelvic floor muscles also known as our sphincter muscles.

·      When these muscles are compromised or injured it can result in loss of bowel and bladder control

·      Specifically, the evidence shows that women that sustain a grade 3 or 4 tear are at greatest risk for developing fecal incontinence at some point in their lives
